Baptism Class Online Registration

Parents, complete this form to register for the Baptism and the Baptism Preparation Classes. This form must be completed and submitted along with a state-issued birth certificate before a Baptismal certificate can be issued. Baptisms will not be scheduled until this birth certificate is brought to class or the parish office.
